    GRANT APPLICATION GUIDELINES
  • SUMMARY

    Gift of Life Donor Program and Donors Are Heroes are seeking to collaborate with houses of faith, faith-based organizations, and service-oriented groups to establish programs that would increase education and donor designations for organ and tissue donation in eastern Pennsylvania, southern New Jersey, and Delaware through grants of up to $5,000 each. Grants can be used to initiate a program or to enhance existing activities of organizations currently promoting organ and tissue donation.     Applicants may receive up to $5,000 which will be determined by the number of people reached (100 minimum). Funds will bepaid at the close of the project once the final report and receipts with appropriate supporting documentation. A portion of the funds may be received up front to help initiate a new program on a case-by-case basis with a written request.  Allow 30 days for processing of funds.  Items purchased with grant monies may not be used for resale.

    Please submit the Grant Application at least two (2) months prior to the date of your program unless granted approval.

    EVALUATION AND MONITORING

    As a condition of accepting the funds, applicants will be required to document project activities by submitting the following:  1) samples of outreach programs 2) periodic information and reports and 3) meet with Gift of Life grant representative for overall educational management and monitoring 4) photos from your events or projects.

    Gift of Life Donor Program will provide educational materials, speakers, and overall guidance to help make the program a success. Gift of Life staff and/or ambassadors will be available to register people as designated organ donors at your event. General surveys (provided by GOL) about organ and tissue donation will also be distributed and collected to aid in the effectiveness of the event.

    APPLICATION ASSESSMENT

    Grant Applications will be assessed using the following criteria:

    1. Describe the project or activity and identify how it will increase education and donor designations for organ and tissue donation.                           

    2. Describe the population to be served and the ways in which efforts will benefit the target population.                   

    3. Indicate specific or measurable outcomes resulting from the projects. How many people will you sign up to be organ donors through your project or event? 

    Note: All programing must have an area with opportunity for audience to register as an organ and tissue donor.

    4. Provide a timeline and budget for the project/activities.

    SAMPLES OF PROJECTS/ACTIVITIES

    The following are samples of projects and activities which have been successful. Please replicate one of the samples below or create your own. Each organization is encouraged to work within a framework that is suitable for its unique community.

    • Breakfast with speakers
    • Luncheon and Presentation
    • Youth Outreach/Roundtable Discussion
    • Concert with speakers
    • Conference Sessions/Panel
    • Community Give-Back
       

    CONTINUED PARTNERSHIP

    Applicants will be expected to partner with Gift of Life Donor Program to promote organ and tissue donation at another event during the year. This may include participating in an event at your House of Faith, Clergy Summit, Donor Sabbath (November), Gift of Life Howie’s House volunteer opportunity, National Donate Life Month (April), National Minority Donor Awareness Month (August), or a local community event.

    As you prepare your budget submissions for review and approval, please note that you are permitted to include staff and volunteers as expense line items. To facilitate the review process, it is essential that each line item includes the titles, names, amounts, and a clear explanation of how each staff member or volunteer contributes to your proposed project.

    Here is the required format for listing staff and volunteers:
